About the Opportunity
As a Health Care Social Worker, you will work within a multidisciplinary team to provide holistic care along the primary to quaternary care continuum. Your role will focus on the relationships between individuals and their environments, addressing social determinants of health through assessment, planning, intervention, and evaluation. You will support patients/clients by building on their existing strengths and resources, fostering emotional, psychosocial, and spiritual well-being.

Key Responsibilities

Conduct biopsychosocial assessments to identify client needs.
Use evidence-based treatment approaches for individuals with severe and persistent mental illnesses, including Psychosocial Rehabilitation and Motivational Interviewing.
Facilitate group therapy sessions.
Provide a client- and family-centered approach to care.
Collaborate with community partners and maintain awareness of local resources.
Maintain up-to-date knowledge of the Diagnostic & Statistical Manual of Mental Disorders.
Travel between sites as required.

About You
We would love to hear from you if you meet the following qualifications:

Education and Certification
Bachelor’s degree in Social Work from a recognized university.
Current registration with the Nova Scotia College of Social Workers.

Experience and Skills

Minimum of 2 years post-BSW clinical experience.
Familiarity with evidence-based treatment approaches and group facilitation.
Effective time management and organizational skills.
Excellent communication skills, both verbal and written.
Knowledge of local community resources is an asset.
Competency in other languages, with French preferred.
