Job Overview:
This is an exciting opportunity to join the RISE Community Health Centre (CHC) team, part of Collingwood Neighbourhood House (CNH), a not-for-profit society serving the Renfrew-Collingwood area of East Vancouver. RISE CHC offers primary care and health promotion services focused on marginalized populations such as newcomers, LGBTQ2SIA+, Indigenous people, isolated seniors, and more. The role involves primary care, health promotion, outreach nursing services, and working with an interdisciplinary team.

Responsibilities:

  • Administer primary care services, including case management, wound management, medication administration, and phlebotomy.
  • Provide mental health and addiction care, as well as chronic and infectious disease management.
  • Perform nursing assessments and develop care plans for priority populations.
  • Engage clients in self-care through health teaching and counseling.
  • Provide culturally safe, trauma-informed care to vulnerable populations.
  • Offer treatment planning, crisis intervention, and case coordination.
  • Work collaboratively with a diverse interdisciplinary team.

Skills and Qualifications:

  • Current practicing registration with the BC College of Nursing Professionals (BCCNP)
  • 2 years of recent related experience
  • Knowledge of Community Health Nursing Standards of Practice
  • Current CPR certification
  • Experience working with marginalized populations and providing trauma-informed care
  • Proficiency in conducting nursing assessments and interventions for common diagnoses
  • Demonstrated ability to perform conflict resolution and apply strategies to address social determinants of health

Education:

  • Bachelor’s degree or equivalent experience
  • Licensure as a Registered Nurse by provincial or territorial authorities

Experience:

  • 2 years to less than 3 years

  • Private clinic
  • Outreach and community settings in Renfrew-Collingwood
