Key Responsibilities

Direct Patient Care:
• Provide compassionate, dignified personal care to patients
• Bathe, dress, and groom patients according to care plans
• Assist patients with toileting needs and supply/empty bed pans
• Weigh, lift, turn, and position patients safely to prevent pressure sores
• Transport patients in wheelchairs or stretchers as needed
• Monitor patient comfort and respond to needs promptly
• Assist with mobility and transfers using proper techniques
• Support patients’ independence while ensuring safety

Health Monitoring and Vital Signs:
• Take and record patients’ blood pressure, temperature, and pulse accurately
• Monitor and document changes in patient condition
• Report any concerns or abnormalities to nursing staff immediately
• Observe patients for signs of distress or health changes
• Follow care plans and protocols precisely
• Maintain accurate patient records and documentation
• Communicate patient status during shift handovers

Nutrition and Meal Support:
• Serve meal trays to patients in timely manner
• Feed patients who require assistance with eating
• Monitor food and fluid intake
• Accommodate special dietary requirements and restrictions
• Encourage adequate nutrition and hydration
• Report changes in appetite or eating patterns
• Assist with meal preparation when required

Environmental Care:
• Make beds and maintain patients’ rooms in clean, organized condition
• Ensure patient environments are safe and hazard-free
• Keep patient living areas comfortable and pleasant
• Maintain infection control and sanitation standards
• Organize personal belongings for easy patient access
• Change linens regularly and as needed
• Monitor and maintain appropriate room temperatures

Activities and Social Support:
• Accompany patients on outdoor recreational activities
• Facilitate patient participation in therapeutic programs
• Provide companionship and emotional support
• Engage patients in meaningful conversations and activities
• Support patient dignity, independence, and quality of life

• Assist with recreational and leisure activities

Additional Patient Care Duties:
• Perform other duties related to patient care and comfort as assigned
