The Barrie and Community Family Health Team (BCFHT) is one of Ontario’s largest family health teams, comprised of a multidisciplinary team of Physicians, Registered Nurses, Nurse Practitioners, Dietitians, Pharmacists, Diabetes and Respiratory Educators, Mental Health Counsellors, Social Workers, Occupational Therapists, and Chiropodists. Our focus is on providing patient-centered, team-based primary health care that promotes self-management, emphasizes health promotion and disease prevention, and enhances care for those with chronic conditions.
Position Overview:
We are seeking a Primary Health Care Nurse Practitioner to join our team on a 0.5 FTE contract basis. The NP will work within a family practice office, alongside other Nurse Practitioners and interdisciplinary team members, to provide comprehensive health care services to patients in Barrie, ON. This role offers an opportunity to collaborate closely with various healthcare professionals, improve patient health and quality of life, and facilitate long-term care planning.
Key Responsibilities:
- Provide primary health care, adhering to standards set by the governing body and aligning with the family health team’s strategic direction.
- Deliver health promotion, disease prevention, acute episodic care, chronic disease management, and palliative care to patients.
- Improve patient access to assessment, diagnosis, and treatment in the community and among target populations.
- Collaborate with an interdisciplinary team, including family physicians, specialists, and community health providers, to coordinate care and treatment plans.
Qualifications:
- Education:Bachelor’s degree (Master’s degree in Nursing is considered an asset).
- Completion of a recognized Primary Care Nurse Practitioner program (NP-PHC).
- Licensing:Current registration with the College of Nurses of Ontario (CNO).
- Completion of an accredited controlled substance course, according to CNO standards.
- Professional Memberships:Membership with RNAO and NPAO is considered an asset.
- Experience:1 to less than 2 years of experience in a similar role.
- Experience working in interdisciplinary teams.
- Familiarity with electronic medical records (Accuro is an asset) and MS Office (Excel, Word, Outlook).
Additional Requirements:
Must adhere to all BCFHT policies and procedures.
Must be legally entitled to work in Canada.
Ability to travel within the community as required.
Work Environment:
Work is performed on-site at various health settings in Barrie, ON. No remote work option is available.
