Humber River Health is seeking a dedicated and compassionate Social Worker to join our Outpatient, Adult Mental Health Urgent Care Clinic. As part of our multidisciplinary team, you will provide psychosocial support, short-term crisis intervention, and therapy to individuals post-discharge from the Emergency Department (ED) and Emergency Psychiatric Unit (EPU). Employment Details

  • Status: Temporary Part-Time
  • Hours: Regular day shifts, some evenings required
  • Location: Wilson Site (Cross-site coverage may be required)
  • Labour Group: OPSEU 590
  • Reporting to: Manager, Mental Health

Key Responsibilities

  • Conduct psychosocial assessments and provide short-term crisis intervention for patients post-ED and EPU discharge.
  • Utilize a variety of therapeutic approaches (e.g., crisis counseling, brief solution-focused therapy) to promote patient stabilization.
  • Offer psychoeducation and consultation services to patients and their families.
  • Collaborate with psychiatrists and the multidisciplinary team to support mental health interventions.
  • Assist in discharge planning to ensure seamless patient transitions.
  • Maintain safety standards for oneself, patients, and the work environment.
  • Participate in reflective practice as per the College of Social Workers’ standards.

Qualifications

  • Master of Social Work (MSW) from an accredited institution.
  • Current membership with OASW and OCSWSSW.
  • Certification in aggressive behavior prevention is an asset.
  • Minimum five (5) years of experience in a mental health setting.
  • Knowledge of community mental health resources.
  • Skills in cognitive behavioral therapy (CBT) and brief solution-focused therapy are an asset.
  • Strong crisis intervention and counseling skills.
  • Excellent knowledge of the Mental Health Act.
  • Strong organizational, decision-making, and interpersonal skills.
  • Multilingual abilities are an asset.
