Join Extendicare (Canada) Inc. as a Registered Nurse in a chronic and long-term care setting and provide high-quality, resident-centered care on our night shift. You will be a clinical leader on the unit, coordinating and delivering nursing care to residents with complex, chronic health needs, while supervising and supporting an interdisciplinary team. This role is ideal for a compassionate, detail-oriented nurse who thrives in a team environment and is committed to maintaining safety, dignity, and quality of life for seniors and long-term care residents.
Work Setting
- Chronic or long-term care facility/centre
- Interdisciplinary team environment (RNs, RPNs/LPNs, PSWs, allied health, physicians)
- Night shift primary coverage, including weekends and statutory holidays as scheduled
Key Responsibilities (Tasks)
- Monitor residents/patients and promptly advise physicians of any changes in condition
- Address and document symptoms and changes in patients’ conditions in accordance with professional and organizational standards
- Assess residents to identify appropriate nursing interventions and priority needs
- Plan, implement, coordinate, and evaluate individualized care plans in consultation with residents and their families
- Collaborate with members of an interdisciplinary health team to ensure comprehensive, person-centered care
- Conduct disease screening and ongoing assessments (e.g., vitals, pain, skin, falls risk, cognitive status)
- Dispense and administer medications and treatments as prescribed by physicians and in accordance with facility policies and scope of practice
- Operate, monitor, and troubleshoot medical apparatus and equipment safely (e.g., pumps, oxygen, lifts, monitoring devices)
- Provide direct nursing care, including wound care, catheter care, palliative/end-of-life care, and chronic disease management
- Ensure infection prevention and control practices are strictly followed and promoted within the unit
- Support rehabilitation and restorative care programs as appropriate to resident needs
- Supervise licensed practical nurses (LPNs), registered practical nurses (RPNs), personal support workers (PSWs), and other nursing staff on shift
- Provide clinical guidance, mentoring, and direction to team members and participate in orientation of new staff
- Maintain accurate, timely nursing documentation and incident reporting
- Participate in quality improvement, risk management, and health policy/program initiatives within the home
- Communicate effectively with residents, families, physicians, and the care team regarding care plans, changes in condition, and goals of care
Education and Experience Requirements
- Bachelor’s degree in Nursing (e.g., BScN or equivalent)
- 1 year to less than 2 years of recent nursing experience (experience in long-term care, geriatrics, rehabilitation, or chronic care is a strong asset)
Licensing and Credentials
- Licensure as a Registered Nurse by provincial or territorial authorities (e.g., registration and good standing with the College of Nurses of Ontario or eligibility to obtain)
- Current CPR/BLS certification (and other mandatory certifications as required by facility policy)
Experience and Specialization
- Infection prevention and control (ensuring adherence to IPAC policies and procedures)
- Rehabilitation and restorative care approaches
- Health policy and program administration (participation in implementing clinical policies, procedures, and quality initiatives)
Work Conditions and Physical Capabilities
- Fast-paced environment with changing priorities
- Standing and walking for extended periods during night shifts
- Physically demanding at times; assistance with resident transfers and use of lifts
- Handling heavy loads and equipment as required, following safe lifting practices
- Repetitive tasks related to medication administration, documentation, and resident care
- Ability to remain alert, focused, and responsive during overnight hours
- Exposure to bodily fluids and various clinical situations; adherence to safety and infection control protocols required
