Job Overview
WELL Health Clinic Network is seeking an experienced Family Doctor to deliver comprehensive primary care in a clinic/private practice environment in North York. This role is ideal for a physician who values continuity of care, evidence-based decision-making, and strong patient relationships. You will manage acute and chronic conditions, provide preventive care and immunizations, order and interpret diagnostics, coordinate referrals and home care supports, and counsel patients and families on wellness and lifestyle changes. Education

  • Degree in medicine, dentistry, veterinary medicine or optometry (medicine required for family physician practice)

Experience

  • 3 years to less than 5 years of relevant experience in family medicine/primary care

Credentials (Required)

  • College of Family Physicians Certification (CCFP)
  • Licentiate of the Medical Council of Canada (LMCC)
  • Licensure by provincial or territorial authorities (Ontario licensure required to practise)

Key Responsibilities (Tasks)

  • Collect medical information from patients, family members, and other medical professionals to support accurate assessment and continuity of care
  • Examine patients’ health conditions to identify diseases, injuries, and risk factors and determine appropriate treatment plans
  • Provide acute care management and ongoing monitoring for chronic conditions
  • Co-ordinate or manage primary patient care, including follow-ups, referrals, and care planning
  • Order laboratory tests, X-rays, and other diagnostic procedures; interpret results and communicate next steps clearly
  • Consult with other medical practitioners to evaluate patients’ physical and mental health and coordinate multidisciplinary care
  • Prescribe and administer medications and treatments in accordance with clinical guidelines and regulatory standards
  • Inoculate and vaccinate patients to prevent and treat diseases; support preventive care initiatives
  • Advise patients on health care, screening, and self-management strategies
  • Help patients develop healthy habits, break old habits, and reshape their approach to wellness through counselling and education
  • Provide counselling and support to patients and their families on a wide range of health and lifestyle issues
  • Supervise home care services when applicable and coordinate with community supports
  • Be the patient advocate by supporting informed decision-making and helping patients navigate care pathways
  • Report births, deaths, contagious diseases, and other reportable conditions to government authorities as required
  • Maintain accurate, timely clinical documentation and protect patient confidentiality

  • Supervise staff in various areas of responsibility as required within clinic operations
