Education

• Degree in medicine, dentistry, veterinary medicine, or optometry (medicine required for physician practice)

Experience

• 3 years to less than 5 years of relevant clinical experience in family medicine/primary care

Credentials (Required)

• College of Family Physicians Certification (CCFP)
• Licentiate of the Medical Council of Canada (LMCC)
• Licensure by provincial or territorial authorities (Ontario licensure required to practice)

Key Responsibilities (Tasks)

• Collect medical information from patients, family members, and other medical professionals to support accurate assessment and continuity of care
• Examine patients’ health conditions to identify diseases, injuries, and risk factors
• Provide acute care management and timely treatment planning for urgent and non-urgent presentations
• Coordinate or manage primary patient care, including follow-up, monitoring, and preventive screening
• Order laboratory tests, X-rays, and other diagnostic procedures to determine appropriate medical treatment
• Consult with other medical practitioners to evaluate patients’ physical and mental health and coordinate referrals when needed
• Prescribe and administer medications and treatments in accordance with clinical guidelines and best practices
• Inoculate and vaccinate patients to prevent and treat diseases; support public health and preventive care initiatives
• Advise patients on health care, wellness, and self-management strategies
• Help patients develop healthy habits, break old habits, and reshape their approach to wellness through counselling and education
• Provide counselling and support to patients and their families on a wide range of health and lifestyle issues
• Be the patient advocate by supporting informed decision-making and navigating care pathways
• Supervise home care services when applicable and coordinate with community supports
• Report births, deaths, contagious diseases, and other reportable conditions to government authorities as required
• Maintain accurate, timely clinical documentation and ensure patient confidentiality and compliance with regulatory standards

• Supervise staff in various areas of responsibility as required (clinic support team and care coordination roles)
