As a Clinical Care Partner , your typical day will impact the following ways :

• In collaboration with the Regional VP, Executive Director, and DOC, provide follow-up and support to all
• care community team members to ensure that care meets and / or exceeds the organization’s and provincial health authority requirements.
• Assist in the development of compliance plans as required, and follow up on compliance action plans in
• consultation with the care community team members.
• Support the development of processes and evidence-based practices. Lead care community and organization-wide improvement processes / projects as directed and in consultation with the Regional VP
• Operations and care community team members.
• Assist and coach care community team members at all levels to improve processes, strategies, and activities concerning care and services through the provision of education and orientation.
• Provide recommendations regarding training and education that would benefit the care communities and the organization as a whole.
• Work in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
• This role requires successful candidates to travel to our long-term care sites to support operations.

Education and Experience Required :

• Must be registered as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario.
• Post Baccalaureate education in Nursing, Health Sciences, or a related field is preferred.
• Preference will be given to candidates with Director of Care experience.
• Certificate of Gerontological Nursing (Canadian Nurses Assoc.) preferred.
• Knowledge and understanding of provincial regulatory requirements and the inspection process.
• 3 years of related management and relevant experience in long-term care.
• Evidence of ongoing professional development and a demonstrated commitment to continuous learning.
• Highly motivated; demonstrates an ability to be self-directed, working independently and as a member of a collaborative team.
• Able to drive and / or travel to long-term care communities – access to a reliable vehicle and valid G licence is required.
