How You Will Make an Impact
Reporting to the Assistant Director, the Registered Social Worker – Intake, Referral, and Transition Coordinator plays a vital role in the HOMES program by ensuring the seamless intake of new tenants, reviewing referrals, and supporting residents transitioning from the HOMES program for a variety of reasons including : aging in place, changes in forensic disposition, & requiring different levels of care. This individual collaborates with all HOMES sites in Hamilton, serving individuals experiencing mental health and addiction challenges, and having a history of homelessness.
Intake and Referral Coordination :
- Process and review referrals for all HOMES sites in Hamilton this will include collaboration with the By-Name Priority List and other referral sources.
- Conduct comprehensive intake assessments to evaluate eligibility and service needs, ensuring a client-focused approach.
- Maintain clear, detailed, and timely documentation of all referrals and intake outcomes.
Resident Transition Support :
- Support residents in transitioning out of HOMES to suitable community-based housing or other services, ensuring they are equipped with necessary resources.
- Develop and implement individualized transition plans in collaboration with tenants and their support teams.
- Facilitate linkages to community services, Long Term Care, and health care providers.
Collaboration and Advocacy :
- Liaise with HOMES site staff, community agencies, and referral sources to ensure efficient service delivery.
- Advocate for tenant needs within the community, ensuring access to essential resources.
- Build relationships with partner organizations to enhance referral and transition processes.
Administrative Responsibilities :
- Maintain accurate and up-to-date records of all tenant interactions, referrals, and transitions.
- Prepare reports and statistics for program evaluation and audit purposes.
- Participate in case conferences and team meetings as required.
General Duties :
- Conduct tours and orientation for potential residents and visitors as needed.
- Assist in program development and evaluation to enhance intake, referral, and transition processes.
- Perform any other duties related to the mission of Good Shepherd Non-Profit Homes as directed by the Assistant Director or their delegate.
In addition, the Registered Social Worker is responsible for participating in Good Shepherd’s Health and Safety programs, procedures, procedures and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to clients, staff and others as well as to the property of the Good Shepherd.
What You’ll Bring to Our Team
Education and Certification : Bachelor’s or Master’s degree in Social Work. Must be a Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
Experience : Minimum of 3 years of relevant experience in intake, referral, case management, or transition coordination, preferably with populations experiencing homelessness, forensic dispositions, mental health, and addiction challenges.
Knowledge and Skills :
- Strong understanding of the principles of Harm Reduction, Housing First, and Psycho-Social Rehabilitation
- Familiarity with community resources, housing services, and health systems in Hamilton.
- Excellent interpersonal, organizational, and communication skills (both written and verbal).
- Ability to work collaboratively within a multidisciplinary team.
- Proficiency in case management software and other relevant computer applications.
Other Requirements :
- Demonstrated cultural competency and sensitivity in working with diverse populations.
- Valid Ontario driver’s license and access to a reliable vehicle.
- Availability for flexible hours to meet program needs.
